Buttocks Lifting

Information in spanish

What is buttocks lifting?

The buttocks are an important aspect of the sexual attraction, especially for some cultures and ethnic groups. Plastic surgery has developed specific treatments to improve the shape and volume of this area of the body. Buttocks surgery may be performed with several different techniques, such as gluteal implants, liposuction and lipoinjection. The choice of the appropriate technique for a patient will take into account among other aspects, the stature, body contexture and weight. Once the surgeon has identified the problem, she may design a treatment to correct or improve shape in this area.

The candidate for buttocks lifting surgery
The buttocks lifting surgery is indicated for patients who want to improve the volume and projection of the gluteal region. Dr. Nieto believes that honest communication is the key to a successful patient-doctor relationship. As a health professional, Dr. Nieto is always willing to listen to your desires, but remember, your health and well-being are priority. Therefore, your doctor may decline to perform a procedure if she considers this is not convenient for your health, or if she identifies patient´s expectations are not real. Dr. Nieto´s purpose is to improve your appearance, though maintaining patient safety is main commitment. The ideal candidate for buttocks lifting surgery is a healthy adult with realistic expectations who wants to improve his/her appearance. The buttocks lift surgery is particularly indicated in the following cases:

  • Congenital atrophy.
  • Trauma.
  • Hypoplasia.

What can I expect from the buttocks lift surgery consultation?
In the first consultation with the physician you should clearly express what you want to change or improve. During the consultation you may feel a little anxious, but remember you are with a health professional that is willing to listen to your desires. A meticulous physical examination is the key to identify the patient´s needs and determine the appropriate treatment.

What aspects are evaluated in the buttocks lift surgery preoperative evaluation?

The physician will evaluate those aspects that can be improved through the several different buttocks lift techniques. Anatomic and aesthetic considerations may include:

  • Lateral hips area.
  • Lateral buttocks area.
  • Hypoplasia.
  • Lateral leg area.
  • Lateral depressions.
  • Infragluteal folds.
  • Supragluteal fossettes.
  • V-shaped crease in the lumbodorsal area.
  • Gluteal projection.
  • Skin laxity.
  • Ptosis and gluteal position.

For determining the ideal projection and shape of buttocks, a classification has been established according to the body contexture, height and weight of the patient. The physical examination will take into account the bone structure, pelvis, thighs, and buttocks shape to identify the type. According to shape, buttocks are classified in:

A-shape:
Better proportioned, this shape is characterized by having more fat in the lateral upper thigh and less fat in the lateral upper hip area.

V-shape:
This shape is reserved to those patients with a tall pelvis, very thin legs, and a tendency toward central obesity.

Square:
The square shaped buttock is characterized by equal volumes at the upper lateral hip area and the lateral leg area with a resulting square appearance.

Round:
The round shape is characterized by having excess fat at the mid buttock area. Patients with round-shaped buttocks have a tendency to be heavier than those with a square shape.

This classification does not correspond exactly to the different anatomic variations, as every person is unique and each treatment must approach to the individual needs. However, this classification may guide the surgical planning for buttock contouring and gluteal augmentation.

By analyzing the anatomic variations in both men and women, and evaluating the different gluteal types, the physician may identify the problem and design a surgical planning. Usually, patients must have a photographic examination which is crucial to help guide the treatment intraoperatively. Once the problem has been established, the physician may recommend a treatment and surgical planning.

What aspects can be improved through the buttocks lifting?

Buttocks lifting may improve and correct the following aspects:

  • Excess perigluteal fat.
  • Excess subgluteal fat.
  • Projection.
  • Lack of volume.
  • Characteristics of aging.

Potential complications

A serious and skilled surgeon will take meticulous measurements before surgery to avoid all possible complications. However, every surgery involves a certain amount of risk. It is important that you understand the risks involved in your particular procedure.

Some of the buttocks lift surgery complications include:

  • Malposition or ptosis.
  • Lateral irregularities.
  • Dehiscence of the incision.
  • Intragluteal infection.
  • Submuscular infection.

Subcutaneous placement of gluteal implants refers a high incidence of long-term complications. Subcutaneous placement involves, among other complications, palpability, visible margins of implant, excess mobility, displacement, ptosis, extrusion, bizarre appearance because of capsular contracture. For these reasons, Dr. Nieto recommend to use the intramuscular plane, in order to avoid these complications.

Since the buttocks are routinely subject to compressive forces, there is risk of implant failure. If it is commonly known the breast implant failure, it is not surprising that because of the position of the implant, gluteal implants involve a major problem. However, it is very important that the patient understands that today´s buttock implants are made of semisolid silicone elastomer, which is much more durable than the breast implant. Also, intramuscular plane placement of the implant reduces risk of implant failure.

General consideration about buttocks lift surgery

  • Every person is unique and each treatment must approach to the individual needs.
  • The surgeon may use different techniques in order to improve the results.
  • The gluteal implant may be placed in the submuscular, intramuscular o subcutaneous plane.
  • Submuscular placement of the implant reduces risk of capsular contracture or implant failure associated with trauma. Despite there is no a common medical criteria about it, it seems that submuscular implant placement offers a significant advantage.
  • Submuscular and intramuscular implant placement is the better alternative for those patients who want to improve volume and gluteal projection.
  • Subcutaneous implant placement involves risk of ptosis, capsular contracture, malposition and seroma.
  • Combination of liposuction and implant placement improve results and postoperative patient satisfaction.
  • Dr. Nieto routinely uses elastic and pneumatic compression devices, as well as anticoagulant medications during and after surgery to prevent venous thromboembolism.

Gluteal size and shape is a very important issue, especially for the Latin-American culture. Frequently, the new reveals sad situations related to plastic surgery procedures performed by non-medical personnel. Surgical practice requires a thorough knowledge of the human anatomy, and the gluteal region includes important structures that can be damaged by unskilled hands. It is always recommended to see a professional plastic surgeon when you are considering plastic surgery.

The Procedure

What type of procedures may be performed through buttocks lift?

As every person is different, according to the gluteal shape, tone and projection the patient may require:

Liposuction
Liposuction is frequently used in those patients who have acceptable gluteal projection and volume, but have excess fat at the paralumbar and supra and infragluteal regions. Liposuction may improve contour by removing those fat deposits that causes disproportion.

Implants
Implants are frequently used in those patients with adequate volume and lack of projection. The round, small-based gluteal implants provide adequate anteroposterior projection with no further side-to-side projection. Liposuction may be used as a complementary procedure. Oval-shaped gluteal implants with a wide base and low profile may improve the shape of buttocks with adequate projection but no volume. Both implants and techniques may vary or being combined to achieve optimum results.

Dermocutaneous adjustment

For some patients with excess skin, particularly obese patients that have lost weight or those post-bariatric patients, it is not possible to correct these defects solely with the use of liposuction or gluteal implants. In these cases, it is necessary to remove the redundant skin before considering other treatments.

Type of implants
